Downscale.Health is a modern, flexible telehealth service led by Australian Nurse Practitioner Justin Black.

We provide private, patient-centred healthcare for adults, families, and children using a mix of clinical consultations (video or phone) and secure messaging — all without needing to visit a clinic.

We offer care across:

How does your model work?

What types of appointments do you offer?

We offer:

  • Video consults (via secure link)
  • Phone consults
  • Secure messaging (inside our clinical platform Halaxy)

Consults are clinical appointments used for assessment, diagnosis, prescribing, or planning

Messaging is used for follow-up, symptom updates, clarification, shared decisions, and check-ins

We blend both — not one or the other — so you can receive true continuity of care, just like in a physical practice.

What clinical software do you use?

We use Halaxy — a secure, Australian-based clinical system that complies with AHPRA and privacy laws. It’s designed for practitioners and ensures all messages, consult notes, scripts, certificates, and referrals are documented in one secure patient record. It’s the same level of documentation and safety you’d expect in a physical clinic.

What are your hours?

Clinician support is available Monday to Friday from 9:00am to 6:00pm, and Saturdays from 10:00am to 2:00pm. You can send messages or book requests anytime, and we’ll respond during these hours. This means you don’t have to fit everything into a rushed 15-minute window — care continues across the week.

Do you treat children?

Yes — we support children of all ages for general health, including fevers, rashes, medical certificates, school or daycare clearance, prescription management, and recovery support. Justin Black brings a strong paediatric emergency background and understands the urgency, reassurance, and flexibility families often need. For weight management, we treat children aged 12 and over in line with best practice, using a supportive and age-appropriate approach.

What is included in the SEVEN plan?

SEVEN includes one phone or video consult plus seven days of follow-up care through secure messaging. If a child develops a rash, or if you’re starting a new medication or need a medical certificate and a clearance, SEVEN covers the consult and the follow-up — without needing a second appointment. It’s perfect for short-term health needs that still deserve continuity.

What is included in the 365 plan?

The 365 Plan includes ongoing consults by phone or video as clinically needed, along with unlimited secure messaging. It is ideal for anyone needing follow-up, medication titration (including GLP-1s, SSRIs, asthma medications), chronic illness support, emotional or behavioural guidance, weight loss coaching, or family health. You don’t pay per visit — care is responsive and continuous.

Can I use this for pathology and results?

Yes. We can request blood tests, imaging, swabs, or other investigations and provide full review, explanation, and next steps. If you’re fatigued and want a full blood work-up, for example, we can manage the entire process from order to result and plan — without requiring another appointment or delay.

What isn’t included?

We do not offer emergency services, and we cannot prescribe certain restricted or S8 medications unless they meet legal and clinical criteria. We cannot replace a physical examination when one is medically necessary, and we refer safely when in-person care is the better option. While we support mental health needs, we do not manage complex psychiatric conditions alone. We also do not treat children under 12 for weight loss, but general health care is supported at all ages.

Is this covered by Medicare?

No, care provided through Downscale.Health is privately billed and not eligible for Medicare rebates. This is a deliberate and necessary decision in response to upcoming changes in national policy.

From November 2025, the Australian Federal Government will introduce new legislation that restricts Medicare billing for telehealth services. Under this change, telehealth clinics will no longer be permitted to bill Medicare unless the patient has been physically seen in a clinic within the previous 12 months. This means that even if a patient has received consistent, safe, and clinically appropriate care via telehealth, their clinician will be unable to continue billing Medicare unless an in-person appointment has occurred.

We believe this policy is short-sighted, inequitable, and detrimental to the thousands of Australians who rely on high-quality virtual care. For many patients — including those who live regionally, parents juggling family responsibilities, individuals with disability or limited transport, or those simply managing full-time work — this restriction places unnecessary barriers between them and the healthcare they trust.

At Downscale.Health, we’ve created a private, transparent model that places the needs of patients above bureaucracy. It allows us to continue delivering consistent, personalised care without being bound to outdated funding models that don’t reflect modern healthcare delivery. It also enables us to remain viable as a small, patient-first business at a time when independent telehealth providers are under threat.

This model allows us to continue offering care that is flexible, responsive, and clinically sound. You’ll still be seen by the same clinician, have access to real follow-up, and receive the level of support that Medicare-funded telehealth models have increasingly failed to provide. We’re not a call centre, and we’re not here for shortcuts — we’re here for continuity, professionalism, and human connection.

We know that for many of our patients, these changes are frustrating and confusing. We’re right there with you. That’s why this model exists — to ensure we can keep showing up for you, even when the system makes that harder. Do I need to download anything?

No. All consults are delivered via a secure link for phone or video, and messaging is conducted through our clinical platform. There are no downloads, apps, or portals required.

How do I pay and cancel?

The SEVEN package is a one-time payment. The 365 Plan is billed monthly and can be cancelled anytime without penalty. You can restart at any time. All payments are processed securely online.

Who will I be speaking with?

All care is provided personally by Nurse Practitioner Justin Black. You will not be passed between random clinicians or call centres. This model is designed to give you ongoing, high-quality care with someone who knows your story.
