The 365 Plan is your all-access pass to personalised healthcare that fits into your real life — not the other way around.

You’ll receive ongoing access to the same clinician, who will support you through a combination of phone or video consults and secure messaging. Whether you’re managing a weight loss journey, starting or adjusting medication, supporting a child’s health, or living with a chronic condition — this plan ensures you’re never left waiting for answers.

There are no per-appointment fees. You don’t need to rebook every time you need help. You’ll have a partner in your care, who checks in, adjusts plans, and responds as things change.

What’s included:

– Ongoing telehealth consults (via phone or video) as clinically required

– Unlimited secure messaging during business hours

– Prescriptions, referrals, medical certificates, pathology/imaging requests

– Dose titration and side effect monitoring for medications such as GLP-1s, SSRIs, asthma management, and more

– Weight loss support across nutrition, medication, mindset, and behaviour

– Support for general health, chronic disease, family care, and mental wellbeing

– Paediatric support (all ages for general health, 12+ for weight management)

– Access hours: Mon–Fri 9am–6pm, Sat 10am–2pm

– Cancel any time

What happens after purchase:

Once you subscribe, you’ll be sent a secure welcome email with your Halaxy intake form and a link to book your initial consult.

From there, your clinician (Nurse Practitioner Justin Black) will get to know your goals, health background, and any current treatments or symptoms. You’ll be able to check in via secure message whenever you need, and book a consult when clinically appropriate — all included in your subscription.

We’ll guide you through everything inside Halaxy — our secure, AHPRA-compliant clinical system. No app downloads, no tech hurdles — just clear, responsive care.
